STANDARD OPERATING PROCEDURE FOR CARRYING OUT EQUIPMENT MAJOR OR MINOR BREAKDOWN MAINTENANCE

Description

1.0    PURPOSE :

               To   describe   a  standard   operating    procedure  for  carrying   out    equipment major / minor breakdown maintenance.

2.0    SCOPE:

    The  standard  operating procedure describes  in detail  the method for carrying out equipment major/minor breakdown maintenance.
    
3.0    RESPONSIBILITY:

    The responsibility of carrying out equipment major/minor breakdown or routine maintenance lies with the staff of rank not lower than supervisor of maintenance.      

The responsibility of preparation, review and approval of the standard operating procedure lies with the Manager – Maintenance, Head of Production  and Head of QAD respectively.

4.0    METHOD:

4.1    On receipt of engineering requisition as shown in Annexure-I in duplicate from the production shift in-charge for carrying out the major/minor breakdown maintenance in the production blocks, the staff not lower than supervisor shall carry out maintenance activities with the help of utility operator. If necessary, work permit / vessel entry permit shall be taken to carry out maintenance job as per the written procedures.
    
    
4.2    After completion of the job, the maintenance person shall hand over the equipment and a copy of engineering requisition to shift in-charge of production department.  He shall file the other copy.       
    
4.3    The supervisor shall enter the details in the "History Card” as shown in 
Annexure-II in case of major breakdown maintenance. The completed requisitions shall be filed separately for breakdown maintenance.
    
4.4    Breakdown maintenance of utilities such as diesel generating sets, refrigeration units, boilers, cooling towers, air compressors etc. will be carried out by the staff not lower than supervisor of maintenance directly without any requisition because these equipments directly come under maintenance department. The major breakdown will be recorded in history card by the rank not lower than supervisor of maintenance. 

4.5    Whenever the major break down / unplanned maintenance occurs for the equipment, the same shall be informed to QA for monitoring and review.
    
5.0    RECORDING:  
                                      
    The responsibility of recording the equipment breakdown maintenance details in history card lies with the staff of rank not lower than supervisor of maintenance.
