3.0 RESPONSIBILITY:
The responsibility of the equipment breakdown investigation lies with the staff of rank not lower than the manager - maintenance.
The responsibility of preparing, reviewing and approving the standard operating procedure lies with the Manager - Maintenance, Head of Production and Head of QAD respectively.
4.0 METHOD:
4.1 Listout the nature of problems which are repeatedly occurring in a particular equipment.
4.2. Listdown all the possible causes for the breakdown .
4.3. Identify the actual cause for the repeated breakdown .
4.4. Eliminate the problem by providing the preventive measures.
4.5 Check for the effectiveness of the measures undertaken.
4.6 Incorporate the effective measures in the preventive maintenance plan.
