Pharmacokinetics

Oral absorption.

Actions

It is a tricyclic antidepressant; this will inhibits the reuptake of serotonin that it blocks the reuptake of noradrenaline and neurotransmitters serotonin almost equally. The onset of action may be longer, up to 8 weeks, in some individuals. It is also effective in migraine prophylaxis, but not in abortion of acute migraine attack.

Dosage/Dosage form

  • Adult: PO- Initial dose: 100 mg per day given in 3 to 4 divided doses. 
  • Pediatric Dose for Primary Nocturnal Enuresis: Initial dose: 25 mg per day orally 1 hour before bedtime.

Therapeutic uses

  • May also be used to manage panic disorders, with or without agoraphobia, as a second line agent in ADHD, management of eating disorders, for short-term management of acute depressive episodes in bipolar disorder and schizophrenia, and for symptomatic treatment of post therapeutic neuralgia.
  • For the relief of symptoms of depression.

Adverse effects/Side effects

Sinus tachycardia, AV/bundle-branch block, postural hypotension, dry mouth, wt loss/gain, constipation, urinary hesitancy/retention, impotence; blurring of vision, exacerbation of glaucoma, liver dysfunction, tremors.

Interaction

  • Reduces hypotensive effects of bethanidine, guanethidine, bretylium, debrisoquine, methyldopa and clonidine.
  •  Severe hypertension with noradrenaline, methylphenidate and adrenaline.
  • Possible serotonin syndrome with MAOIs, separate admin by 3 weeks.
  • Increased plasma levels and effects with quinidine, cimetidine, SSRIs, propafenone, flecainide.
  •  Reduced plasma levels with barbiturates, phenytoin.
  • May increase effects of anticholinergic drugs.
  • Severe orthostatic hypotension with altretamine.
  • Drowsiness and impaired performance may cause with alcohol.

Contraindications

Post MI, heart block/arrhythmias; mania; porphyria; severe hepatic impairment.

Precautions

Children, cardiac disease; elderly, Epilepsy; pregnancy and lactation; DM; angle-closure glaucoma; prostatic hyperplasia; phaeochromocytoma. Monitor for increased suicidality during early treatment. Withdraw gradually.
