Pharmacokinetics

  • Distribution: 10-49% protein binding
  • Metabolism: Hepatic( Partially metabolized through liver)

Actions

Amrinone is a positive inotropic cardiotonic with vasodilator properties, phosphodiesterase inhibitory activity, and the ability to stimulate calcium ion influx into the cardiac cell.

Dosage/Dosage form

  • Loading dose: 750 mcg/kg by slow inj over 2-3 min
  • Max cumulative dose: 10 mg/kg in 24 hr.
  • Maintenance dose: 5-10 mcg/kg/min by infusion.

Therapeutic uses

Used in the treatment of congestive heart failure.

Adverse effects/Side effects

GI disturbances, nausea, thrombocytopaenia, vomiting, hypotension, hypersensitivity, chest pain, myositis, nail discoloration, vasculitis, Inj site pain. Potentially Fatal: Cardiac arrhythmia, hepatotoxicity.

Interaction

  • Concurrent usage with disopyramide may lead to severe hypotension.
  • Chemical interaction with precipitation occurs with amrinone and dextrose or furosemide. Thus drug mixing should be avoided.

Precautions

  • Furosemide should not be administered into an IV line delivering Amrinone.
  • Amrinone must be diluted in a 1/2 normal saline or normal saline; dextrose solutions should not be used for dilution. 
  • Obstructive pulmonary or aortic disease, hypertrophic cardiomyopathy; platelet counts and liver function should be monitored; BP and heart rate monitored during drug administration; fluid and electrolyte balance should be maintained; Pregnancy and lactation. Protect amp from light.
