Description

Zebeta (Bisoprolol) is used for treating high blood pressure and heart failure

Generic Name

Bisoprolol fumarate- belongs to a group of medicines called beta-blockers.

Chemical names

Bisoprolol; Zebeta; Bisoprolol hemifumarate; Fondril

Brand names

Bebedol, Bisbeta, Biselect, Bislod-AM, Bisocar-HT, Bisod, Cadrol, Concor, Concor-AM, Concor-Cor, Corbis, Corbis-H, Corbis-H, Lodoz, Qualiz, Zabesta, Zabesta-XLO.

IUPAC name

1-[(propan-2-yl)amino]-3-(4-{[2-(propan-2-yloxy)ethoxy]methyl}phenoxy)propan-2-ol

Pharmacokinetics

Oral absorption with 80% bioavailability and 20 % undergoes first pass metabolism.

Actions

Bisoprolol selectively blocks catecholamine stimulation of β1-adrenergic receptors in the heart and vascular smooth muscle. This result in a reduction of heart rate, systolic and diastolic blood pressure, cardiac output, and possibly reflexes orthostatic hypotension. At higher doses competitively may block β2-adrenergic receptors in vascular and bronchial smooth muscle causing bronchospasm and vasodilation.

Dosage/Dosage form

Doses of Bisoprolol fumarate ranged from 5 to 60 mg per oral.

Therapeutic uses

For management of heart failure, angina pectoris, and mild to moderate hypertension and for secondary prevention of myocardial infarction (MI).

Adverse effects/Side effects

Giddiness, headache, faitgue, bradycardia. Nausea, vomiting, diarrhoea or constipation, stomach discomfort, mild ocular stinging, decreased sexual ability, photophobia, keratitis, dysponea, insomnia, cold extremities, drowsiness, hallunications,  mood alterations, severe bronchospasm, hypotension, hypoglycaemia, orthostatic hypotension, orthostatic hypotension, orthostatic hypotension, bradyarrhythmias. 'Rebound phenomenon' leading to unstable angina or MI on sudden withdrawal.

Interaction

  • Decreased effect with AI & Ca salts, barbiturates, cholestyramine, NSAIDs ampicillin, rifampicin.
  • May mask tachycardia from hypoglycaemia induced by oral hypoglycaemics and insulin.
  • Enhances the action of Ca antagonists, anesthetic agents, digitlis, clonidone, hypoglycaemia agents & NSAIDs.
  • May increase the effects of drugs which slow AV conduction, α blockers and α- adrenergic stimulants.

Contraindications

Low cardiac output & uncompensated cardiac failure, 1st degree heart block, sinus Bradycardia, bronchospasm; cardiogenic shock, severe haemorrage. Pregnancy.

Storage

Keep it out reach of children.
