Pharmacokinetics

Actions

Brimonidine significantly lowers intraocular pressure with minimal effects on cardiovascular and pulmonary parameters. It lowers IOP by reducing aqueous humor production.

Dosage/Dosage form

Instill 1 drop of a 0.2% solution bid-tid.

Therapeutic uses

Used to treat open-angle glaucoma or ocular hypertension.

Adverse effects/Side effects

Oral dryness; fatigue or drowsiness; burning and stinging; conjunctival follicles, headache, blurring of vision; foreign body sensation, eyelid oedema, ocular allergic reactions, ocular pruritus, corneal staining or erosion, photophobia, eyelid erythema, ocular ache and pain, tearing; ocular dryness, upper respiratory symptoms; conjunctival oedema, dizziness, blepharitis, ocular irritation; GI symptoms; asthaenia; conjunctival blanching, abnormal vision and muscular pain.

Interaction

  • Possibility of an additive or potentiating effect with CNS depressants e.g. alcohol, barbiturates, opiates, sedatives or anaesthetics.
  • Caution when used with ophthalmic and systemic β-blockers, cardiac glycosides or antihypertensive drugs.
  • TCAs can affect the metabolism and uptake of circulating amines.

Contraindications

Hypersensitivity. Patients receiving MAO inhibitor therapy

Precautions

  • Patients wearing soft contact lenses should wait for at least 15 min after instilling brimonidine tartrate before inserting soft contact lenses.
  • CV disease.
  • Renal or hepatic impairment.
  • Use with caution in patients with depression, cerebral or coronary insufficiency, Reynaud’s phenomenon, orthostatic hypotension or thromboangitis obliterans.
  • Pregnancy, lactation.
