Actions

Thiazides such as hydrochlorothiazide promote water loss from the body (diuretics). They inhibit Na+/Cl- reabsorption from the distal convoluted tubules in the kidneys. Thiazides also cause loss of potassium and an increase in serum uric acid. Thiazides have been shown to prevent hypertension-related morbidity and mortality although the mechanism is not fully understood. 

Dosage/Dosage form

  • Edema- The recommended dose is 25-100 mg once daily or in divided doses.
  • HTN- The recommended dose is 12.5 to 50 mg once daily. 

Therapeutic uses

For the treatment of high blood pressure and management of edema.

Adverse effects/Side effects

Volume depletion& electrolyte imbalance, dry mouth, thirst, lethargy, drowsiness, muscle pain, & cramps, hypotension, hypersensitivity reactions, thrombocytopenia, pancreatitis, jaundice, fatigue, impotence, hyperglycaemia, anorxia, gastric irritation, nausea, vomiting, constiption, diarrhoea, sialadenitis, dizziness, raised Ca concentration. Hypersensitivity reactions.

Interaction

  • Diuretic- induced hypokalaemia may increase the toxicity of digitalis glycosides and risk of arrhythmias when used with drugs that prolong QT inteval.
  • May increase serum concentrations of lithium when used together.
  •  Concurrent use with alcohol, barbiturates & opoids may enhance diuertic - enhanced orthostatic hypotension.
  • Antihypertensive effect may be reduced by corticosteriods, NSAIDs or carbenoxolone. May enhance the nephrotoxicity of NSAIDs.
  • Enhnaces neuromuscular blocking action of competitive muscle relaxants.
  • Impaired control of diabetics by oral hypoglycaemic agents.

Contraindications

Severe hepatic & renal impairment, Addison's disease, preexisting hypercalcaemia, anuria, sulphonamide allaergy. Pregnancy & lactation.

Store it at controlled room temperature (20° to 25°C).

Precautions

  • Existing electrolyte disturbances, hepatic cirrhosis, severe heart failure, oedema, elderly, renal impairment.
  • Monitor for signs or fluid & electrolyte disturbance.
  • DM, hepatic impairment, gout, hyperlipidaemia, hyperuricaemia, hypercalcae,ia, ECG; LVH (Extrasystoles). May axacerbate or activate SLE in susecptible patients.
