Better absorbed orally, hepatic metabolism.

Actions

Hydromorphone is a hydrogenated ketone derivative of morphine that acts as a narcotic analgesic. It has a shorter duration of action than morphine but it is better absorbed orally than morphine. Its primary actions of therapeutic value are analgesia and sedation. In addition to analgesia, alterations in mood, euphoria and dysphoria, and drowsiness commonly occur. 

Dosage/Dosage form

Dose is based on patient’s condition. It comes as a tablet, syrup to take by mouth. It also comes as a rectal suppository.

Therapeutic uses

For the relief of moderate to severe pain such as that due to surgery, cancer, trauma/injury, or burns.

Adverse effects/Side effects

Respiratory depression, weakness, agitation, headache, tremor, nervousness, uncoordinated muscle movements, depression, dreams, floating feeling, muscle rigidity, muscle tremor, tingling, blurred vision, night blindness, double vision, disorientation, visual disturbances and increased intracranial pressure. Flushing of the face, chills, fast/slow heart rate, faintness and low/high blood pressure. Light-headedness, dizziness, sedation, nausea, vomiting, flushing, anxiety, dry mouth and itching.  Shortness of breath, increased sweating, poor blood circulation, palpitations, respiratory arrest and shock. 

Interaction

This medication can interact with atropine, buprenorphine, belladonna, butorphanol, darifenacin, dicyclomine, glycopyrrolate, ipratropium, MAOIs, Nalbuphine, scopolamine and benztropine.

Contraindications

Hydromorphone is contraindicated in patients with narcotic analgesics allergy including morphine, codeine and methadone.

Precautions

  • This drug is not prescribed for asthmatic attacks, COPD, spinal curvature, history of head injury or brain tumor, hepatic or renal disorder, thyroid dysfunction, sulfite allergy, hypotension, prostate disorders, pancreatitis, Addison’s disease, seizures and bowel obstructions patients. 
  • Hydromorphone can cause habit-forming. Hydromorphone should not be prescribed to individuals with history of alcohol/substance abuse.
