Dosage/Dosage form

PO- Initial: 5 mg once daily. Maintenance: 10 mg/day. Max: 20 mg/day.

Therapeutic uses

  • Angiotensin II formation will prevents, through that it decreases blood pressure.
  • This medication is an ACE inhibitor, prescribed for high blood pressure. 

Adverse effects/Side effects

Dizziness, headache, fatigue, GI and taste disturbances, chest pain, persistent dry cough and other upper respiratory tract symptoms, paraesthesias, skin rash, angioedema, photosensitivity, renal impairment, hyperkalaemia, hyponatraemia, blood disorders, proteinuria, palpitations, stomatitis, tachycardia, pancreatitis, alopecia, cholestatic jaundice, muscle cramps, mood and sleep disturbances, impotence.

Potentially Fatal: Excessive hypotension, which could result in MI or stroke in patients with ischaemic heart disease or cerebrovascular disease.

Interaction

  • Reduced antihypertensive effects or additive adverse renal effects with NSAIDs.
  • Additive hyperkalaemic effects with potassium supplements, potassium-sparing diuretics, and other hyperkalaemic drugs.
  • Marked hypotension with diuretics, other antihypertensives, alcohol, and other agents that lower BP.

Contraindications

  • Aortic stenosis or outflow tract obstruction; renovascular disease; ascites.
  • Pregnancy and lactation.

Precautions

  • Monitor for proteinuria.
  • Before and during therapy assess renal function
  • Generalised atherosclerosis, peripheral vascular diseases, heart failure, idiopathic or hereditary angioedema, patients likely to be salt or water depleted.
  • Conduct regular WBC counts in SLE and scleroderma patients.
  •  Liver cirrhosis.
  • Discontinue diuretics 2-3 days before starting therapy and resume later if required.
  •  Elderly.
