Actions

Isosorbide Dinitrate is a moderate to long acting oral organic nitrate used for the relief and prophylactic management of angina pectoris. It relaxes the vascular smooth muscle and consequent dilatation of peripheral arteries and veins, especially the latter. Arteriolar relaxation reduces systolic arterial pressure, systemic vascular resistance, and means arterial pressure.

Dosage/Dosage form

20-120 mg daily in divided doses for the management of angina. 

Therapeutic uses

For the prevention of angina pectoris due to coronary artery disease.

Adverse effects/Side effects

Hypotension, tachycardia, flushing, headache, dizziness, palpitation, syncope, confusion. Nausea, vomiting, abdominal pain. Apprehension, restlessness, weakness and vertigo.

Interaction

  • Vasodilatory effect may be reduced with dihydroergotamine.
  • Increased hypotensive effects with alcohol or vasodilators.
  • Marked orthostatic hypotension may occur when used with calcium channel blockers.
  • Significant hypotension may occur with phosphodiesterase-5 inhibitors.
  • Ergotamine effects may be enhanced.
  • Effectiveness may be reduced by sublingual form of disopyramide.

Contraindications

Severe hypotension or anaemia, hypovolaemia, heart failure due to obstruction, or raised intracranial pressure due to head trauma or cerebral haemorrhage.
