Pharmacokinetics

It is absorbed through buccal mucosa, metabolized through lungs, liver and kidneys. Excreted unchanged in urine.

Actions

Nicotine is a stimulant drug that acts as an agonist at nicotinic acetylcholine receptors. Nicotine exerts two effects, a stimulant effect exerted at the locus ceruleus and a reward effect in the limbic system. 

Dosage/Dosage form

  • Patch: One for 16 or 24 hours hours daily applied to dry, hair, free skin on the hip, trunk or upper arm for smoking cessation.
  • Inhalator: Inhale with 10 mg/cartridge nicotine
  • Nasal spray: 1 spray of 500 mcg administered into each nostril
  • Chewing gum: 2-4 mg gum

Smoking cessation 

Adverse effects/Side effects

Nausea, myalgia, hiccups, abnormal hunger, dysmenorrhoea, rash, cold, headache, and flu like symptoms.

Interaction

Nicotine increases hemodynamic & AV blocking effects of adenosine 

Contraindications

Contraindicated during pregnancy and lactations, contraindicated in geriatrics, children’s, and in patients of acute MI, cerebrovascular accident, severe cardiac arrhythmias, unstable or worsening angina pectoris, active temporomandibular joint disease.
