Pharmacokinetics

Oral absorption with 99% bound to protein (Albumin). Hepatic metabolism, excreted via urine.

Actions

It is a racemic mixture of two active isomers, used in the prevention and treatment of thromboembolic disease including thromboembolism, pulmonary embolism and venous thrombosis it is also used to prevent ischemic stroke in patients with atrial fibrillation (AF).

Dosage/Dosage form

5 mg daily

Therapeutic uses

For the treatment of pulmonary embolism, retinal vascular occlusion, cardiomyopathy, flutter, atrial fibrillation, transient cerebral ischaemia, cerebral embolism, arterial embolism and thrombosis.

Adverse effects/Side effects

Hypersensitivity, rash, alopecia, diarrhea, drop in haematocrit, Haemorrhage, purple toes syndrome, skin necrosis, jaundice, nausea, vomiting, hepatic dysfunction, pancreatitis, increased LFT.

Interaction

  • Avoid major changes in dietary intake of Vitamin K (green vegetables eg broccoli, spinach). Increased anticoagulant effects with chamomile, cranberry, gingko biloba, alfalfa, black cohosh, dong quai, celery, clove, capsicum, fenugreek, omega 3 triglycerides and ginger.
  • Reduced anticoagulant effect with St John's wort, Panax ginseng, coenzyme Q10, mistletoe.

Contraindications

  • Contraindicated in pregnancy.
  • Hypersensitivity
  •  Haemorrhagic tendencies or blood dyscrasia; recent surgery; peptic ulcer; severe hypertension; bacterial endocarditis; psychosis; cerebrovascular disorders; senility; pericarditis; aneurysms; eclampsia; pericardial effusion; pre-eclampsia; threatened abortion; alcoholism; severe renal and hepatic impairment

Precautions

  • Heparin induced thrombocytopenia, DVT, infectious disease, disturbances of intestinal flora, surgery or trauma, indwelling catheters, hypertension, Vitamin K, Vitamin C, protein S or C deficiency.
  • Discontinue therapy if necrosis develops.
  • Elderly, alcoholics, lactation
  • Periodic determination of PT/INR. Patient to report any signs of bleeding, symptoms of blood clot and avoid activities or sports that can cause injuries.
  • Purple toes syndrome may occur between 3-10 wk after start of therapy.
  • An INR>4 represents higher risk of bleeding while INR <2 generally represents insufficient anticoagulation.
