1.0 PURPOSE:
To describe a standard operating procedure (SOP) for vessel entry for attending any break down work.
2.0 SCOPE:
The SOP describes in detail the procedure to be followed for taking vessel entry permit for attending any break down work.
3.0 RESPONSIBILITY:
The responsibility of taking vessel entry permit as per the SOP lies with the staff of rank not lower than operator of production department.
The responsibility of preparing, reviewing and approving the SOP lies with the Officer (Production), Head of Production and Head of QAD respectively.
4.0 METHOD:
4.1 The vessel in which maintenance people have to enter to attend the breakdown work has to be inspected thoroughly by Maintenance Department, Production Department, Electrical Department and Safety Department in the following lines.
4.1.1 PRODUCTION DEPARTMENT:
The vessel has to be thoroughly washed from all chemicals / solvents and should be made suitable for entry.
4.1.2 MAINTENANCE DEPARTMENT:
The vessel has to be isolated (Dummy / disconnect the pipe lines connected to the vessel after taking clearance from the Production Department).
4.1.3 ELECTRICAL:
The vessel has to be disconnected from all electrical connections
4.1.4 SAFETY DEPARTMENT
Safety department should play major role in giving vessel entry permit by providing all necessary safety appliances implementing other safety procedures.
4.1.5 HEAD OF THE FACTORY
Head of the factory should physically inspect the site and should be satisfied that all the departments have taken necessary precautions for vessel entry.
A format has to be maintained as shown as Annexure - I.
5.0 RECORDING:
The responsibility of maintaining the format lies with the staff of rank not lower than Production Officer of the Production Department.
